Community Roots Integrative Health and Primary Care is currently accepting new-patient registration requests in preparation for our Fall 2026 opening.

To get started, please complete our secure, HIPAA-compliant new-patient registration through OptiMantra. After submitting your registration, you’ll receive an email with access to your Community Roots New Patient Intake Form and secure Patient Portal link, where you can complete the required consent forms.

Once your information has been completed and reviewed by our clinical team, we’ll contact you regarding the most appropriate next steps for scheduling.
