Do I have to be a Primary Care patient of Community Roots to book a wellness service such as women's health, men's health, weight management, or IV Therapy?No. You do not have to be an established primary care patient of community routes to be seen for wellness services. These services are available to patients with outside primary care providers as well.
Do you see children at this office?
Unfortunately, no.
At this time, Community Roots provides care for adults ages 18 and older. We do not currently offer pediatric care services.
Are you accepting New Patients?Yes. Community Roots is currently accepting new adult patients ages 18 and older. New patients can begin by completing the intake form, and our office will follow up with next steps for portal setup, forms, and scheduling.
Can I use my insurance for wellness services?
Some wellness visits or services may be eligible for insurance coverage depending on the reason for the visit, your specific insurance plan, and medical necessity. Other wellness services, including certain elective or cash-based treatments, may not be covered by insurance.
Community Roots will review your visit type and insurance information ahead of time so we can help clarify what may be billed to insurance and what would be considered self-pay.
What makes Community Roots different?Community Roots was built to bring healthcare back to what it should feel like: personal, thoughtful, and centered around the patient. We focus on longer visits, real conversation, continuity with the same provider, and care that looks at the whole person, not just a diagnosis or a rushed checklist.
Our goal is to combine traditional primary care with a more relationship-based, integrative approach, giving patients space to be heard, understood, and involved in their own care.
Do I have to become a Concierge member or pay the monthly fee to become a patient at Community Roots?
No. You do not need to become a concierge member or pay a monthly fee to receive primary care at Community Roots.
Community Roots offers traditional insurance-based primary care, and concierge membership is an optional enhanced-access service for patients who want additional time, communication, continuity, and care coordination.
