Depending on your chosen plan, concierge-style membership includes:

• Priority scheduling, including reserved same/next-business-day appointment availability when clinically appropriate

• Longer appointment time allocations than standard scheduling

• Enhanced direct communication with Community Roots through secure messaging

• Expanded access to your provider for non-urgent questions and care navigation

• Enhanced care coordination with specialists, facilities, pharmacies and outside providers

• Personalized annual health/wellness planning session that is separate from the insurance-covered annual physical

• Proactive health goal tracking and periodic check-ins

• Review and organization of outside medical records

• Assistance navigating specialist care, referrals and outside testing

• Priority prescription and routine administrative request handling

• Reduced or waived fees for certain non-covered administrative services—forms, letters, record requests, etc.

• Member discounts on clearly non-covered cash wellness services such as IV hydration or certain wellness programs, in compliance with patient’s insurance requirements.

Insurance may still be used for covered services depending on your plan structure, while membership helps support enhanced access and deeper personalized care.
