Community Roots Integrative Health and Primary Care is currently accepting new-patient registration requests in preparation for our Fall 2026 opening.

To get started, please complete our secure, HIPAA-compliant new-patient registration through OptiMantra. After submitting your registration, you’ll receive an email with access to your Community Roots New Patient Intake Form and secure Patient Portal link, where you can complete the required consent forms.

Once your information has been completed and reviewed by our clinical team, we’ll contact you regarding the most appropriate next steps for scheduling.

Community Roots offers a predictable pay structure for adult patients 18+ who don’t have insurance coverage. New patient visit must be completed prior to scheduling any other appointments.

60 Minutes

Initial new patient comprehensive visit/intake to establish care within the practice.

Extended Office Visit

40 Minutes

Appointment is for covering more than one healthcare concern.

For established primary care patients at Community Roots. Must have had new patient visit prior to booking.

Routine Office Visit

25 minutes

Office visit to cover one health care concern.

For established primary care patients at Community Roots. Must have had new patient visit prior to booking.

Acute/Sick Visit

Cough/cold, UTI, STI, rashes, dental infections, abscess, lacerations, conjunctivitis, etc

For established primary care patients at Community Roots. Must have had new patient visit prior to booking.
